Overview
How many times has a patient in your office wanted to try lifestyle change before medication? Do you have the resources (time, community contacts, etc.) to support the patient in adopting healthy lifestyle behaviors? So much of chronic care management is dependent upon nutrition, physical activity, mental well-being, and other lifestyle factors. Are patients and employees well-equipped to make lasting changes that can impact their health before turning to medications and treatments that are so often at their easy disposal? 

These questions are key to understanding how primary care clinicians serve as a driver of lifestyle behavior change and how employers can partner with them to increase employee buy-in and motivation to use lifestyle as a first line of defense for chronic disease. Join us on December 4th as Dr. Padmaja Patel, President of the American College of Lifestyle Medicine and long time primary care physician, shares her experience delivering innovative care models that integrate lifestyle medicine with value-based primary care, and details what it takes from clinicians, purchasers, and health plans in order to cultivate a primary care system that supports and promotes healthy patient lives.

Community Connections is an event series designed to strengthen shared knowledge and understanding across health care stakeholder groups and advance primary care in the St. Louis region. A common table for local practicing clinicians, health care purchasers, health plans, medical group and other community leaders, Community Connections is an opportunity to share experiences, perspectives, and ideas for strengthening primary care. 
Objectives
  1. Learner will acknowledge that the principles of lifestyle medicine, when incorporated into patient care, lead to increased satisfaction for both the clinician and the patient, as well as improved health outcomes.
  2. Learner will receive the method and means to administer patient care with a greater focus on the principles of lifestyle medicine.
  3. Learner will be able to apply strategies to implement lifestyle medicine principles into their own practice.
  4. Learner will be equipped to influence the community attitude and approach to the practice of lifestyle medicine in primary care, using the levers afforded to them in their role within the health care system.
  5. Learner will listen and respond to the priorities, perspectives, and challenges from other learners and community members regarding the implementation of lifestyle medicine in primary care to gain greater shared understanding of local needs.

How can we make St. Louis a national leader in primary care?

The Midwest Health Initiative is convening local practicing clinicians, health care purchasers, health plans, medical group, and other community leaders to learn together and discuss ways to advance primary care in St. Louis.  Community Connections is an event series designed for the region’s health care stakeholders to build relationships and exchange perspectives, while amplifying the voice of primary care clinicians to community leaders. Each event focuses on a timely topic related to primary care where attendees learn from an expert in the field and hear perspectives from community members.

The December 4th event, Stopping Chronic Disease Where It Starts, will focus on the importance of primary care in driving healthy lifestyle change to fight chronic disease. Do clinicians have the resources to support patients in adopting healthy behaviors, and are patients well-equipped to make lasting changes to improve their health? Grievance Policy & Procedure

The monitoring and assessment of compliance with these standards and the SLU CME Program Mission is the responsibility of the CME Program Director in consultation with the CME Program Accrediting Director & CME Chairperson, and the Continuing Education Committee. While Saint Louis University CME Program attempts to anticipate issues and assure fair treatment for all participants to conduct and experience valid and successful CME activities, there may be occasional issues which require intervention and/or action on the part of the CME Program. To register a complaint or grievance with the Saint Louis University CME Program:

Submit a written, detailed complaint or grievance to [email protected]. Complaints will be reviewed by the Program Director and addressed or transferred to the appropriate level for review:

  • Answers and/or actions taken to rectify complaints will be documented and sent to those that provide contact information.
  • If appropriate, complaints will be reviewed at the CME Committee Meeting for members to discuss and find solutions to the area of complaint.
  • Compliance issues will automatically be moved up to the Accrediting Director for immediate review.
