The conference sessions will focus on current and emerging best practices for the management of obesity, diabetes, non-alcoholic fatty liver disease, and cardiovascular disease:

- Rising incidence and prevalence of Obesity and T2DM
- Parallel increase in prevalence of NAFLD; up to 40% of the US population affected
- Introducing current and new treatment strategies to address these multiple overlapping disorders (with common pathogenic mechanisms)
- Pediatric Metabolic Diseases
- Polycystic Ovary Syndrome
- Impact of metabolic disorders on Renal outcomes
- Impact of metabolic diseases on COVID outcomes

These conditions are seen by all physicians and are often not captured or under-diagnosed. Our aim is to bring attention to these common disorders and need for early identification of those at highest risk.

Objectives
By attending this program, participants will be able to:

  1. Summarize the interactions of metabolic disorders with various organ systems including the kidneys, lung, heart, brain, ovaries, liver, skin

Management of Metabolic Risk Factors in the Pre- and Post Transplant Setting
8:00AM - 8:30AM
Kamran Qureshi, MD

Presence of Obesity is more frequent among organ transplant candidates and after transplantation. This trend is noted in parallel to the rise in MASLD as an indication for liver transplant. Obesity is assessed by BMI in transplant population; however, BMI alone is not an ideal parameter in patients with liver cirrhosis. Obese cirrhotic population have multiple metabolic co-morbidities and need special multi-disciplinary management. Sarcopenia as well as co-morbidities of metabolic syndrome are important determinants of long-term outcomes in transplant patients. Endo-bariatrics, along with rapidly evolving medical options of weight loss, as well as sleeve gastrectomy, are promising modalities in patients with cirrhosis before transplant or even after transplant.

Objectives:
  1. Describe the burden of obesity in transplant population.
  2. Explain the impact of obesity on outcomes related to transplant.
  3. Review the options of weight management in the transplant setting.

Joel Schilling, MD, PhD

In this talk I will summarize what is known about the links between metabolic liver disease and cardiovascular complications.  This will include a discussion about cellular and molecular mechanisms and possible strategies for therapies.  I will conclude by highlighting important unresolved issues and future directions in the field.

Objectives:
  1. To appreciate the overlap of metabolic dysfunction associated steatotic liver disease with cardiovascular disease.
  2. To recognize how changes in liver biology can influence cardiovascular complications.
  3. To realize the therapeutic options for cardiometabolic disease that may improve outcomes via effects on liver and the cardiovascular system.

10:30AM - 11:00AM

Targeting Type 2 Diabetes Mellitus in the Metabolic Syndrome
11:00AM - 11:30AM
Scott Isaacs, MD, FACP, FACE

I will be presenting on the evaluation and management of type 2 diabetes in the context of insulin resistance and the metabolic syndrome. I will present a case of a patient with diabetes and other cardiometabolic conditions.  There is overlapping pathophysiology of insulin resistance with type 2 diabetes, chronic kidney disease, metabolic dysfunction-associated steatotic liver disease, cardiovascular disease, dyslipidemia, hypertension, and obesity. Targeting type 2 diabetes in the metabolic syndrome combines glycemic control along with the simultaneous management of these common comorbidities.

Objectives:
  1. Appreciate the overarching pathophysiology of insulin resistance in type 2 diabetes, obesity and cardiometabolic disease.
  2. View treatments for type 2 diabetes beyond the glycemic effect and realize the impact on cardiometabolic risk reduction.
  3. Assess the impact of therapeutic weight loss on type 2 diabetes and cardiometabolic comorbidities.

Objectives:
  1. Describe the mechanistic relationship between adiposity, lipid metabolism and type 2 diabetes.
  2. Apply data driven approaches to the treatment of type 2 diabetes.
Time Restricted vs. Intermittent Fasting Effects on Metabolic Health
