Physical, Occupational and Speech Therapy roles in Early Consciousness Disorder Post Stroke

Nicole Neveau, PT, DPT, NCS, CCI

This presentation explores the role of therapy in early post-stroke acute care. We will definite vegetative state, minimally conscious minus state and minimally conscious plus state, how these terms apply to patients after a stroke and what early interventions are best practice in acute care. We will look at standardized outcome measures used for determining coma recovery stage and how these measures help our patients transition from critical care to post-acute recovery.

Objectives:
  1. Participants will be able to distinguish between a vegetative state, minimally conscious minus state and minimally conscious plus state in a patient post-stroke with the use of standardize assessments.
  2. Participants will be able to define the role for physical, occupational and speech language therapy within critical care units for patients after a stroke who are presenting in a minimally conscious state.
  3. Participants will be able to formulate an appropriate discharge recommendation for post-acute care recovery pathway in a patient presenting in a minimally conscious state.
ICU Clinical Coaching Program
10:35AM - 11:05AM
Krista Wilds, BSN, RN, CCRN, SCRN

I’m going to walk you through the how and why of the ICU Clinical Coaching Program and the impact it has had on nurses, patients, and families. The Covid-19 pandemic led to an unprecedented nurse staffing crisis when a large number of seasoned nurses suddenly left bedside clinical practice. This meant that we lost most of our mentors who were responsible for growing and developing novice nurses. In their wake were young and relatively inexperienced nurses who were asked to step up as preceptors before they were ready. The Clinical Coaching program was developed to provide orientation structure, clarity of expectations, advanced critical care education, emotional support, and resources for both the new nurse and the preceptor.

Objectives:
  1. Describe how a phased orientation program can facilitate developmentally appropriate progression from novice to competent critical care nuse.
  2. Explain how the role of a Clinical Coach is unique and separate from that of a perception and serves to promote the professional development of both the preceptor and the orientee.
  3. Discuss how the concepts of Clinical Pattern recognition and mindfulness-based attention training and tools such as the "Start your Shift Safely" checklist support beginners as they learn to think like an expert and respond to VUCA environments with a sense of urgency.
